Ayushman Card: Under Ayushman Bharat Pradhan Mantri Jan Arogya Yojana, Dr. Khubchand Baghel Health Assistance Scheme, Ayushman Card will be made free of cost through Choice Centers, Chief Medical and Health Officer while giving information said that Choice Center ( Carry documents and cards to Chioce centers)
(Ayushman Card) The date for making Ayushman card to be made under Ayushman Bharat Pradhan Mantri Jan Arogya Yojana, Dr. Khubchand Baghel Health Assistance Scheme has been extended. Ayushman card can now be made free of cost till 31 October through choice centers. Some documents have to be carried along to make Ayushman card.
It is worth noting that earlier the last date for making free Ayushman card was 30 September, which has now been extended to 31 October. Chief Medical and Health Officer Ambikapur Dr. PS Sisodia informed that free Ayushman cards are being made in the choice centres.
Such families who have not got Ayushman card made for themselves and all the members of their family, they can get Ayushman card made by taking active ration card and Aadhar card.
Free treatment up to 5 lakh and 50 thousand
Under Ayushman scheme, eligible beneficiaries can avail up to 5 lakhs and Dr. Khubchand Baghel health assistance scheme eligible beneficiaries can avail up to 50 thousand in registered health institutions.
These documents are necessary to make a card
1. Ration and Aadhar card required.
2. One passport size photograph each of all the family members.
3. Mobile number must be mentioned during registration.
