First of all you have to open the official website of Uttar Pradesh ( https://fcs.up.gov.in ). After this, you have to go to the homepage and click on Form Download. You will see a dropdown list in front, where you have to click on Application Forms. After this, you have to go to the city and rural one and select your Application Form link. Now you have to download the application form visible in front and fill your details in it. After this you have to go to your regional CSC center and submit the form. Remember that there should be no mistake in the form or else the form will be rejected. If the form is correct then your ration card will be generated in a few days. After this, Rs 5 to Rs 45 have to be deposited and when the field verification process of 30 days is completed, then your ration card is issued.
